PILOT ERROR IN COFFS CHOPPER CRASH

The Australian Transport Safety Bureau has found pilot error was the cause of a helicopter crash at Coffs Harbour Airport earlier this year.
The pilot was found to have altered his pre-flight checks and as a result, forgot to switch-on the fuel valve.
The Osborne Aviation helicopter took off shortly after 7am on April 17, with the pilot and one passenger on board.
10 metres above the ground, the engine failed and the Bell Jet Ranger crashed.
The pilot suffered serious injuries, and the tail rotor was sheared off in the accident.
