WorkCover is investigating an accident at a Glen Innes correctional facility which resulted in the death of a 58-year-old man.
Initial enquiries indicate the male truck driver was tarping a load of wood when an unmanned forklift rolled down a slope crushing him between the vehicles.
WorkCover is conducting an extensive investigation, as a report is prepared for the coroner.
